  • · Proof of the new accommodation
  • · Completed change-notification form
  • · A separate notification for each family member; one parent may report the address for a child under 15

What to do step by step

    Report only after the actual move, but no later than 3 working days afterwards

    Submit at KACPU / a Ministry office, by post, datová schránka, email with a recognised electronic signature, or through a representative

    If sending by post, never send the original travel document

    Check Portál občana to confirm the new address is actually recorded
